The Additional Medicare Tax: What It Is and Who Pays It

Who benefits from the Additional Medicare Tax?

The funds collected through Medicare taxes are used to cover medical expenses for eligible individuals, including hospital stays, doctor visits, and prescription drugs. On top of the standard Medicare tax payments, high-income earners pay the Additional Medicare Tax of 0.9%. The IRS has several rules to determine who qualifies based on filing status and income limits. So, married taxpayers filing together have a different income threshold than separate filers. Employers must meet their responsibilities for the Additional Medicare Tax or face a tax penalty. Line 5d has been added to Form 941, Form 941-PR and Form 941-SS.

Tax withholding and estimated tax

Taxpayers who earn above $200,000, or $250,000 for married couples, will pay an additional 0.9% toward Medicare. The Additional Medicare Tax helps fund some elements of the Affordable Care Act. This includes the premium tax credit and other features. You determine your SE tax using the IRS Schedule SE on either Form 1040 or 1040-SR. You may then deduct the employer-equivalent amount of your self-employed tax — half of the total amount — from your gross income. By paying this tax while you are working, you are funding the services you may encounter once you become a Medicare beneficiary.

You must receive written notice from the IRS to file Forms 941-SS instead of Form 944 before you may file these forms. For more information on requesting to file Forms 941-SS, including the methods and deadlines for making a request, see the Instructions for Form 944. If you’re married and filing separately — you’re responsible for paying the additional Medicare tax if your income threshold is above $125,000. If you’re married and filing jointly — the income threshold increases to $250,000. The additional Medicare tax is 0.9%, but it doesn’t apply to everyone like standard Medicare tax does.

Noncash wages and RRTA compensation are subject to Additional Medicare Tax withholding, if, in combination with other wages, or with other compensation in the case of RRTA compensation, they exceed the $200,000 withholding threshold. This section sets forth the legal authorities requiring federal agencies to certify to the IRS the amount of federal income taxes withheld from amounts paid to U.S. Government employees working in, as well as federal civilian and military pensioners residing in, American Samoa, the Commonwealth of the Northern Mariana Islands (CNMI), and Guam.

An employee who doesn’t have a social security card should apply for one on Form SS-5, Application for a Social Security Card. 179, Guía Contributiva Federal para Patronos Puertorriqueños, will no longer be available after 2023. Instead, information specific to employers in the U.S. territories and agricultural employers will be included in Pub. The Additional Medicare Tax, while seemingly straightforward, involves various nuances that are critical for taxpayers to understand.

Under the rules discussed below, the only difference between farm and nonfarm workers’ employment tax deposit rules is the lookback period. Therefore, farm and nonfarm workers are discussed together except where noted. If you’re not sure your total liability for the current quarter will be less than $2,500 (and your liability for the preceding quarter wasn’t less than $2,500), make deposits using the semiweekly or monthly rules so you won’t be subject to an FTD penalty.

  1. Apply for an employer identification number (EIN) online.
  2. Both employees and employers need to be aware of their responsibilities regarding this tax.
  3. For each whole or part month that a return isn’t filed when required, there is a failure-to-file (FTF) penalty of 5% of the unpaid tax due with that return.
  4. All employees, employers and self-employed workers in the United States, regardless of citizenship or residency status, must pay Medicare taxes to fund the Medicare Hospital Insurance program.

For information on reporting back pay to the SSA, see Pub. Determine the value of noncash pay (such as goods, lodging, and meals) by its fair market value. Except for farmworkers and household employees, this kind of pay may be subject to social security, Medicare, and FUTA taxes. If you incorrectly treated an employee as a nonemployee and didn’t withhold social security and Medicare taxes, you’ll be liable for the taxes. See Treating employees as nonemployees in section 2 of Pub. 15 for details on Internal Revenue Code section 3509, which may apply.

However, if the employee asks you for Form W-2, furnish it within 30 days of the request or the last wage payment, whichever is later. An employer reimburses an employee by applying the overwithheld amount against taxes to be withheld on future wages. File a Form 943 each year for all taxable wages paid for farmwork. You may report the wages of household workers in a private home on a farm operated for profit on Form 943. Don’t report wages for farmworkers on Form 941-SS or Form 944.

The employer then continues to withhold it each pay period until the end of the calendar year. There’s no employer match for Additional Medicare tax. You’re responsible for the employer social security tax on wages and tips until the wages (including tips) reach the wage base limit. You’re responsible for the employer Medicare tax for the whole year on all wages and tips. Tips are considered to be paid at the time the employee reports them to you. Deposit taxes on tips based on your deposit schedule as described in section 8.

Named the Additional Medicare Tax, it adds an additional 0.9% tax on top of the 1.45% employees have to pay. Employers are not required to match the amount and the employee is responsible for the entire amount of the extra tax. If you are self-employed, your Medicare taxes are included in your self-employment tax. Withhold social security and Medicare taxes from statutory employees’ wages if all three of the following tests apply. A marriage of two individuals is recognized for federal tax purposes if the marriage is recognized by the state, possession, or territory of the United States in which the marriage is entered into, regardless of legal residence. The Additional Medicare Tax is intricately linked to the Affordable Care Act (ACA).

Employers pay another 1.45%, for a total of 2.9% of your total earnings. Self-employed people pay the entire 2.9% on their own. The Affordable Care Act added an extra Medicare surtax for people with higher incomes starting in January 2013. TAS is an independent organization within the IRS that helps taxpayers and protects taxpayer rights. Their job is to ensure that every taxpayer is treated fairly and that you know and understand your rights under the Taxpayer Bill of Rights. The IRS uses the latest encryption technology to ensure that the electronic payments you make online, by phone, or from a mobile device using the IRS2Go app are safe and secure.

All cash wages that you pay to any employee for farmwork are subject to social security and Medicare taxes if either of the following two tests is met. Report tips and any collected and uncollected social security in boxes 1, 5, 7, and 12 on Forms W-2AS, W-2CM, W-2GU, or W-2VI and on Form 941-SS, lines 5b, 5c, and, if applicable, 5d (Form 944, lines 4b, 4c, and, if applicable, 4d). Don’t include any uncollected Additional Medicare Tax in box 12 of Form W-2. Report a negative adjustment on Form 941-SS, line 9 (Form 944, line 6), for the uncollected social security and Medicare taxes. The table in section 12 shows how tips are treated for FUTA tax purposes. Back pay, including retroactive wage increases (but not amounts paid as liquidated damages), is taxed as ordinary wages in the year paid.

However, if you anticipate liability for Additional Medicare Tax, you may request that your employer withhold an additional amount of income tax withholding on Form W-4. The additional income tax withholding will be applied against your taxes shown on your individual income tax return (Form 1040 or 1040-SR), including any Additional Medicare Tax liability. Code section 7654 and 48 U.S.C. section 1421(h) provide that the U.S. Government is required to cover over to the Treasury of Guam the federal income taxes withheld on amounts paid to military and civilian employees and pensioners who are residents of Guam.

If there is a balance due on the return, you can e-file and e-pay in a single step by authorizing an electronic funds withdrawal (EFW) from your bank account while e-filing. Don’t use EFW to pay taxes that are required to be deposited.
